Ordering Instructions

Ordering from Bentley Sales Company is as easy as 1-2-3!

1. Print out the order form or use a piece of paper.
2. Fill out the order form listing:
   • Your name, home address, ship to address (if different from home address)
   • Method of payment - check, money order, Visa, MasterCard, Discover, or UPS C.O.D.
   • Quantity of models, model numbers, descriptions, and prices
   • Find shipping charge on charts below order form and enter on order form.
   • Illinois residents - add 8.0% sales tax
   • To find your total cost, add price of models, sales tax (for Illinois residents) and shipping charge
3. Send your order to us by:
   • Mail - to Bentley Sales Company, 642 Sandy Lane, Des Plaines, IL 60016. Include your order form, check, money order, or charge information.
   • Telephone (847/439-2049) or FAX (847/439-2071) - Please have your Visa, MasterCard, or Discover Card ready.
